Module C01 Health and Wellbeing of the Remote Worker

Aim

This module will introduce students to the remote medical environment and in particular to the specific issues faced by establishing a medical care service in such.

Learning Outcomes

  1. Demonstrate understanding of the specific challenges faced by planning, installing and running a remote medical service.
  2. Demonstrate awareness of unique ailments and considerations facing the remote care practitioner.
  3. Evaluate the practical constraints placed upon the service by budget, client focus or local conditions.
  4. Demonstrate an understanding of the moral, legal and ethical reasons for promoting general and occupational health to the remote site worker.
  5. Demonstrate an awareness of the various, international, national and industry bodies and the role they play in the delivery of remote health care.
  6. Demonstrate an understanding of where the provision of health care falls in overall project Health and Safety Management Systems.
  7. Demonstrate an awareness of the elements considered essential to the provision of remote site health care, from conception to field execution, (e.g. Pre deployment, Emergency Response Planning, health care provision and facilities and medical evacuation methods).
  8. Demonstrate a working knowledge of health impact, social impact, health risk assessments and their impact on field operations.
  9. Demonstrate an understanding of the importance of safe food, water and hygiene standards in the remote setting.

Module C02 Occupational Health in the Remote Setting

Pre-requisites: Module C01

Aim
Much of remote medical care relates to the workplace. This module introduces some of the key concepts of occupational medicine and investigates the risks, hazards, measures and preventative measures related to industrial medicine in a remote location.

Learning Outcomes

  1. Demonstrate an understanding of occupational health and its relation to the remote environment.
  2. Demonstrate knowledge and understanding of occupational health hazards and their management within the remote environment.
  3. Demonstrate an awareness and understanding of the tools available to measure and control health risks within the workplace.
  4. Demonstrate an understanding of how the remote medical practitioner can support the reduction of occupational related ill-health through practical application of appropriate procedures.
  5. Understand and demonstrate the importance of clear, concise, and confidential medical records within the occupational health field.

Module C03 Communication in the Clinical Consultation

Pre-requisites: Module C01

Aim
This module provides an introduction to communication and the consultation process. It considers communication in general, along with some basic psychological research findings and the concept of consultation models. It also examines specific aspects of the consultation, such as difficulties including, breaking bad news and dealing with different cultures. It also explores the use of the computer in the consultation and associated clinical decision support techniques and software.

Learning Outcomes

  1. Be able to critically evaluate various models of the consultation.
  2. Be able to apply a selected consultation model to one's own consultations to enable evaluation.
  3. Have knowledge of factors that facilitate effective giving of bad news.
  4. Have knowledge of cultural factors in the consultation.
  5. Have knowledge of how to use the computer appropriately in the consultation.
  6. Be aware of the main findings concerning Clinical Decision support research.
  7. Gain practical skills in applying various consultation models to a videoed consultation.
  8. Develop analytical skills in reflecting upon the application of various consultation models to assess your own, and recorded consultations.
  9. Gain practical skills in applying various consultation models to your own consultation.
  10. Be aware of core psychological findings that are relevant when considering the consultation.

Module C04 Telemedicine

Pre-requisites: Module C01

Aim
The main feature of remote medicine is the geographical and transport distance from consultant level diagnosis and advanced care. This module introduces the concept of using technology to facilitate access to specialist care.

Learning Outcomes

  1. Demonstrate understanding of the underlying technology principles of a telemedicine system (TMS).
  2. Demonstrate awareness of the main approaches to providing remote solutions to deliver patient care.
  3. Evaluate the process and business considerations when defining and implementing a TMS in the remote medical care context.
  4. Evaluate a technological or process solution to improve patient care in the remote setting.

Module C05 Dive Medicine

Pre-requisites: Module C01

Aim
This module introduces the key concepts of dive medicine including the medical observation of divers, diseases and organisms specific to the marine environment and the management of medical conditions and emergency situations.

Learning Outcomes

  1. Demonstrate an understanding of the physics, physiology and pathophysiology relevant to the diving environment.
  2. Demonstrate knowledge of the equipment and procedures used in diving.
  3. Demonstrate an understanding of how diving accidents happen, how to identify them and how to manage them.
  4. Demonstrate an understanding of how the remote medical practitioner can help to prevent a diving accident by giving sound advice on safe environmental conditions, monitoring of divers, care of the healthy diver and fitness to dive.

Module C06 Tropical Medicine

Pre-requisites: Module C01

Aim
This area of medicine deals with diseases that are unique to or more difficult to control in tropical areas. Managing the disease can be complicated when combined with poor hygiene, restricted access to medical care, poverty and lack of infrastructure.

Learning Outcomes

  1. Demonstrate awareness and use of intelligence sources in tropical medicine.
  2. Recognise and manage the signs and symptoms of common tropical diseases and their vectors.
  3. Appraise the treatment, prevention and social issues surrounding tropical disease.

Module C07 Aeromedicine

Pre-requisites: Module C01

Aim
This module investigates the branch of medicine focused on the movement of patients by air and on the clinical, aviation and logistics issues that ensure safe and effective missions.

Learning Outcomes

  1. Demonstrate understanding of the specific challenges and risks faced in the transport of patients by air.
  2. Demonstrate awareness of patient physiology and pathology and medical equipment and logistics unique to the air medical environment, and the importance of pre-flight patient assessment.
  3. Evaluate and resolve the practical constraints placed upon air medical transport by cost, security, and logistics concerns.
  4. Learn to develop effective planning tools for air medical transport, and demonstrate an understanding of the decision process in choosing the aircraft provider, the medical and nursing team, and the route, timing and destination of air medical transport.
  5. Demonstrate an awareness of the role of the patient and his/her family, the treating doctor, the receiving doctor, the airport authorities, aircrew and cabin crew, Customs and Immigration, and the "customer" in the safe transport of our patient. Revise the basics of loading, moving and securing patients to compensate for special risks and constraints that apply in the air medical context.
  6. Demonstrate an informed understanding of portable medical diagnostic, monitoring and transport equipment.
  7. Demonstrate awareness of changes in the patient's physiology, primarily relating to oxygenation and circulation during various phases of flight, how to manage them proactively, and how to document air medical missions.
  8. Be aware of the basics of moving a patient by "commercial flight". Especially the expectations of airline staff, fellow passengers, the limitations of the cabin environment including seating by class and movement within the cabin, and the limited medical equipment and supplementary oxygen supplies allowed on an airliner. Learn how to respond effectively and professionally to calls for medical assistance in an in-flight emergency when travelling as a passenger.
  9. Be aware of the basic precautions to take when transporting patients by rotary wing (helicopter), and the special role of medical and nursing staff in helicopter transports for primary retrieval missions (SAR - "search and rescue"), in mountainous terrain, offshore, at night, and at altitude.
  10. Be aware of the extra precautions both in terms of crew and equipment required in critically ill patients, including the very young, the elderly, multiple trauma, and psychiatric cases.
  11. Have an intelligent awareness of how to behave in the air medical environment while executing professional medical and nursing responsibilities, subordinate to the "pilot in command" and airline, security and airport authorities. Understand the impact aviation security has on specific equipment and specific conditions (for example, sharps, batteries, fluids, psychiatric conditions etc.) and know how to deal with limitations imposed by security regulations.
  12. Look ahead to the future of air medical transport for patients benefit.

Module C08 Trauma and Medical Emergencies

Pre-requisites: Module C01

Aim
This module looks at trauma and medical emergencies and their treatment. It also looks at professional planning and management of the emergency.

Learning Outcomes

  1. Demonstrate an ability to consider coping strategies to deal with and to plan for acute emergencies.
  2. Demonstrate the ability to identify the major risks in your own setting and the resources available to help to identify them. The resources will be limited or non-traditional.
  3. Demonstrate knowledge of means of prevention of acute medical and trauma emergencies.
  4. Understand the current advances in trauma care and relate them to the remote or offshore setting.
  5. Understand the management of specific trauma injuries and discuss specific examples related to your own setting.
  6. Recognise the life threatening medical and surgical emergencies and treatment plans.
  7. Develop the ability to keep current in medical advances.

Module C09 Incident and Response Planning

Pre-requisites: Module C01

Aim
This module is all about being prepared for any emergent situation. It provides an overview of risk assessment, incident planning and the management of incidents of any size. The focus will be on the pre-planning and the activation and coordination of the planned response.

Learning Outcomes

  1. Examine general and specific risks and the inherent hazards associated with managing an incident.
  2. Develop an incident, evacuation and hazard plan.
  3. Demonstrate understanding of the structure of an activated response plan and the duties and chain of command in each branch.

Module C10 Evidence and Research in Remote Medicine

Pre-requisites: Module C01

Aim
This module investigates the identification, evaluation and use of evidence for best practice. It also introduces key concepts of the research process and the generation of knowledge. These concepts will be applied in the remote medical care setting.

Learning Outcomes

  1. Identify and critically appraise appropriate sources of research information.
  2. Critically evaluate the relevance of an evidence based approach for remote medical practice.
  3. Demonstrate understanding of the research process.
  4. Evaluate the use of different research techniques used in health related research.
  5. Critically analyse the application of a research approach to remote medical care.
