This statement builds on the previous 2017 version and incorporates current evidence, clinical developments, and expert consensus from across pre-hospital, obstetric, and emergency care. The updated recommendations address key areas including physiological considerations in pregnancy, trauma management, resuscitation principles, airway care, aortocaval decompression, haemorrhage control, use of tranexamic acid, and appropriate patient positioning. Guidance is also provided for the rare but time-critical decision-making required in cases of maternal cardiac arrest, including considerations around resuscitative hysterotomy.
The statement aims to support clinicians working across the pre-hospital environment — including paramedics, doctors, retrieval teams, critical care providers, and multi-agency responders — by offering clear, practical principles to improve safety and outcomes for both mother and fetus.
